(a) The Company is in the process of dissolving;
(b) The Company has filed with the Minnesota Secretary of State a notice of intent to dissolve;
(c) Such notice of intent to dissolve was filed on July 1, 2016;
(d) The address of the office to which written claims against the Company must be presented is:
